KeepMeHome’s Assisted Care Management program enables clients to live at home by providing them the care and services where they need it, when they need it. These services can also be provided in assisted living or retirement centers.

Assisted Care Management takes a comprehensive approach in assisting your practice in the management of clients that you might act for as conservator or responsible party.

The Assisted Care Management program will either provide or coordinate services. Our services include our care and companion aides, housekeeping and maintenance for the client’s home, transportation escorts to the client’s medical appointments, medical appointment making. Notification services to your practice as to the client’s status and wellbeing are also available.

Our office is open from 6 a.m. to 10 p.m. every day of the year, with on-call personnel available after those hours. KeepMeHome can assist your group and give you comfort and peace of mind that your client’s health is well cared for.
